to your personal record.
This
form will generate a message to the NCE office, which
will subsequently update your record. The change will NOT occur
immediately.
The
only required information is your
NAME and EMAIL. Otherwise, please fill in only the
fields that are CHANGES to your record.
It
is NOT necessary to fill in all of the boxes, BUT YOUR
RECORD CANNOT NOT BE UPDATED IF YOU FAIL TO PROVIDE YOUR
NAME AND EMAIL INFORMATION
